Container handlers are mobile, rubber-tired, machines that are used to load and stack standard containers at a variety of shipping terminals. The machines resemble oversized forklifts but are equipped with shipping container lift spreaders instead of forks. The machines are often equipped with sophisticated computer systems that inform the operator which container needs to be lifted as well as its destination, whether it is a truck, train, or storage stack. Harmless Appearing, Smooth Rotating Mixer Shaft Causes Serious Finger Amputation Injuries

I recently finished working on a case where an operator received a right hand injury when an unguarded, rotating mixer shaft caught his gloved hand between the right index finger and middle finger, twisting it around the shaft causing the index finger to separate from the hand and the middle and ring finger to be dislocated. The mixer contained an unguarded rotating shaft, which created an uncontrolled motion hazard when rotating at speeds up to 450 RPM. (more…)
Injury on Power Buggy

When an operator is thrown from a ride-on vehicle when it tips over, such as a mud buggy, serious injury or death can result. In one such case we investigated, the issue centered on the maintenance of the vehicle, specifically whether a maladjusted hydrostatic drive caused the braking mechanism to malfunction. (more…)
Wind Turbine Rotor Overspeed at Wind Energy Park

A wind turbine was being erected as part of a small wind energy park. The wind turbine tower and the nacelle housing the generator were successfully installed. The three-bladed rotor assembly was installed and connected to the nacelle as the last major component of the wind turbine. (more…)

Crawler Crane Calamity at Cement Plant

A production building was being constructed at a cement plant. A large crawler crane was being used to install the pre-assembled metal wall framing of the building. The weights and lift radii of the four wall framing sections, along with the rigging spreader and other lift equipment, were within the rated capacity of the crane. (more…)
Forklift Falls off Loading Dock

A forklift fell off the loading dock of a warehouse as it was in the process of entering a semi-trailer that was being loaded. The forklift fell because the semi-trailer and the connected truck rolled away from the edge of the loading dock as the forklift passed over the dock leveler into the trailer. The repeated braking forces of the forklift as it carried loads onto the trailer caused the trailer to move away from the loading dock. The semi-trailer wheels were not chocked to prevent movement away from the loading dock. (more…)
Winter Safety Tips for Portable Electric Generators

During winter ice storms, power to residences is often lost and homeowners revert to using gasoline powered portable electric generators. It is often thought by some homeowners that leaving a generator outside in the severe weather may damage it or affect its performance and therefore resort to bringing the unit inside the shelter, typically, the garage. This is particularly hazardous when the generator is run for an extended period of time as carbon monoxide (CO) poisoning from the toxic exhaust gas can cause death. In addition, other hazards including electric shock, electrocution, fire and burns potentially can occur. (more…)
Mobile Crane Ground Support

Mobile cranes depend on stability through their outriggers. These are the four “legs” that are deployed onto soil or other working surfaces adjacent to the crane. When a mobile crane is set up at a site, the outriggers are deployed by a hydraulic mechanism that extends the four outriggers beyond the crane body and then jack the crane free of its wheels so that it is supported by the outriggers only.
