  1. Crane Pile Driving: Who Owns This Loss?

    The boom of a hydraulic crane was bent while removing temporary sheet piles at a construction site.  A vibratory hammer had been placed at the top of the sheet piles to both drive and remove the piles. The vibratory pile driving placed no significant loads on the crane boom, as that operation relied on the weight of the sheet piles and the vibration of the hammer to sink the piles into the soil.  The vibration of the hammer and the lifting force of the crane boom were needed to remove the sheet piles from the soil. In addition, ANSI machine tool standards further address equipment hazards and risks that cannot be eliminated by design. In one case I investigated, the depth of that critical risk assessment became an important factor in a claim involving a machinist blinded while using a computer numerically controlled (CNC) machining center.   3. Fire Losses Involving Improper Construction and Installation of Fireplaces and Chimneys

    On a cold winters night a family is gathered in the living room of their home enjoying the warmth of their fireplace.  After using the fireplace all day, they go off to bed to a nice restful sleep.  However, they are soon rudely waked by the sound of a smoke alarm in the home.  Rushing out into the cold night they can see smoke, but no flame in the home.  Outside, they can see that the chase around the fireplace chimney is burning.  The fire department is called and thankfully extinguishes the fire before it does widespread damage, however the family must leave the home for several months as repairs are made.   4. Mobile Crane Ground Support

    Mobile cranes depend on stability through their outriggers. These are the four “legs” that are deployed onto soil or other working surfaces adjacent to the crane.  When a mobile crane is set up at a site, the outriggers are deployed by a hydraulic mechanism that extends the four outriggers beyond the crane body and then jack the crane free of its wheels so that it is supported by the outriggers only.     The design and installation of gas piping systems in homes and commercial buildings is guided by a set of regulations called NFPA 54 — the National Fuel Gas Code.   8. Explosion Investigations Uncover Answers

    The cause of an explosion can result from a chain of events in some cases. In many residential explosions and resulting fires, the facts can point to a number of potential causes. Certified fire and explosion investigators, like Warren engineers, have the experience and training to uncover and evaluate scenarios with high degrees of certainty. In some cases, like one residential explosion we investigated, the chain of events can start days before the loss.
